R6 Regional Council is accepting applications for a full-time case manager (CM) to join our Aging Services team for in-home service programs. The CM will provide case management for the Veterans Directed care program. This position will also function as the Ombudsman which monitors long term care facilities for implementation of resident rights.
Applicants must have a four-year degree or expected completion within six months of hire. A CCM or SSW certification is not required but preferred. This is a full-time position with full benefits. Starting salary is $20.00 to $24.00 per hour depending on certifications and experience. Please call 435-893-0747 for additional information.
