The R6 Regional Council, based in Richfield, Utah, invites qualified and licensed Occupational Therapists (OTs) to provide professional home assessment and consultation services under the Aging in Place program. This program seeks to empower older adults to remain safely and independently in their homes through targeted home modifications and supportive services. R6’s mission is to deliver high-quality, client-centered aging services across Juab, Millard, Piute, Sanpete, Sevier, and Wayne counties.
The selected Occupational Therapist will work collaboratively with R6’s Program Manager.
Responsibilities include:
● Review and collaborate with the Program Manager on each initial home inspection, reviewing documentation and discussing how to serve the client best using the Home for Life Design assessment tool (which will be provided).
● Conduct follow-up home assessments to evaluate the effectiveness of installed modifications, recommend any necessary adjustments, and provide client education.
● Work in collaboration with Program Manager to assist in ensuring all work is compliant with ADA and local building standards as related to accessibility and aging in place.
● Providing education and training to clients on the proper use and maintenance of installed accessibility features and equipment.
● Coordinating with Program Manager to ensure timely and effective service delivery.
● Supporting community outreach efforts to promote awareness of the program among eligible populations and community stakeholders.
QUALIFICATIONS: Minimum required qualifications include:
● Current Occupational Therapist license in the State of Utah.
● Strong interpersonal, documentation, and communication skills.
● Available to travel to and service residences in all six counties (Juab, Millard, Piute, Sanpete, Sevier, and Wayne).
COMPENSATION: Competitive hourly or per-assessment compensation
● Employment Status: Part-time or Independent contractor.
